What You Will Do:
Our case managers, who we call Supports Planners, play a significant role in the lives in individuals with complex medical needs, behavioral health needs, and/or those with disabilities. In this role you are at the center of coordination and planning while working collaboratively with your clients, their circle of support, and service providers within the community. Our clients value their partnership with our Supports Planners as they collaborate on the development of person-centered goals, and gain access to the best services and community resources to support their independence in their local community. This position is a home-based position, and requires visits to client homes, assisted living facilities, and other service provider locations within Maryland.

The hourly rate for this position is $26.44-$27.40/hour with the opportunity for quarterly success payments and mileage reimbursement.
