Who You Are

If you are a registered nurse (RN) or licensed social worker (LMSW, LCSW-C) and have experience in case management/care coordination, home health, school nursing, or pediatrics, this might be the organization for you! What You Will Do

As a Clinical Care Coordinator, you will provide day-to-day care management and coordination services within the REM Program (Rare and Expensive Case Management). Your clients will be recipients of Medicaid, most of whom are under age 21, and have complex medical needs. Partnering with their family/caregivers, you will work directly with clients on creating an individualized plan of care with goals to help them remain independent in the community setting of their choice. This position is a hybrid of teleworking and client visits in the community setting including client homes, facility settings, school/day programs, and hospitals.
