What You Will Do
As a Clinical Care Coordinator, you will provide day-to-day care management and coordination services within the Model Waiver Program. Partnering with families and caregivers, you will work directly with clients on creating an individualized plan of care with goals to help them remain independent in the community setting of their choice. This position is a hybrid of teleworking and client visits in the community setting including client homes, facility settings, school/day programs, and hospitals.

The salary for this role is $70,000-$72,000 and comes with all of our full-time benefits. This position requires the CCM certification within two years of employment, with a $1500 stipend upon completion.
